Once a referral has been made the following will be sent to the parent:

    • Procedural Safeguards Notice
    • Authorization for Release of Information
    • Developmental History Form
    • Medical Release of Information (If necessary)
    • Teacher observation form to be given to the child’s teacher

Once the referral documentation has been completed by the parent and returned to the Department of Special Programs, the Local Education Agency reviews the submitted documentation and determines if there is sufficient evidence to support conducting a comprehensive psycho-educational evaluation. The parent is invited to a meeting to discuss the need for evaluation and the District’s decision to evaluate or not.  If the District determines that no evaluation is indicated, the parent is provided justification for this decision in writing through prior written notice. If the District determines that further evaluation is needed to determine if the child is eligible as a child with a disability, written, informed consent is obtained from the parent and the evaluation process begins.

If you have any questions, please contact the Department of Special Programs by calling the Child Find Secretary at 803-641-2428, ext. 12796 or emailing SPED@acpsd.net.
