For out of district and overnight field trips, each student will need to fill out a field trip medical form.  If your student has medication that will be brought on the trip, the following forms will need to be filled out...

Medication Permission Form: One form for every medication brought needs to be filled out completely and signed by parents.  If it is a prescription medication, the permission form needs to be signed by a physician as well OR a copy of the prescription with the physician's signature can be obtained from the pharmacy that filled it.

For emergency medications (inhaler, epi-pen, diabetic monitoring supplies) that your child will keep on their person; a Self-medicate/self monitor packet will  need to be filled out and on file with the school.  There are 3 forms; parent, student and health care provider that will need to be filled out.
